A tele-MFM budget line and partner letter for your Rural Health Transformation application

Three pieces a hospital, clinic or network can adapt for its state's application: a scope of work for maternal-fetal medicine consults and ultrasound interpretation by telehealth, a budget line that separates grant-funded availability from claim-funded visits, and a support letter from the clinical partner. Copy what fits, change what doesn't, and check it against your state's notice.

Sample scope of work

Maternal-fetal medicine consultation by telehealth for pregnant and postpartum patients of [Facility], available 24 hours a day, seven days a week, for routine consults, high-risk pregnancy co-management with the patient's local clinician, and urgent consults from the emergency department and, where [Facility] delivers, the labor unit. Interpretation of obstetric ultrasound studies acquired by [Facility]'s sonographers, with a signed report to the ordering clinician within 24 business hours for routine studies and within two hours for STAT studies. Written protocols and worksheets for each study type, and training for sonographers, nurses and coordinators in the telehealth workflow. Coordination with [Facility]'s transfer and referral pathways so that a finding that needs a higher level of care reaches it, and one that doesn't stays local.

Keep the grant against what a claim doesn't pay for. Where your state funds services insurance doesn't cover (provider payments are capped at 15% of the state's award), say which ones and for how long, and show the transition. Year-one money has to be committed by October 30, 2026 and spent by September 30, 2027.
